To apply for an exemption for the carriage of an assistance dog you will need to complete the following forms:


When submitting your completed form, this must be accompanied by the appropriate medical assessment form completed by your GP or Consultant who is treating you for the condition and who has full access to your medical history.

Based on the evidence provided the Council’s Licensing Officers will determine if you should be exempt from these duties. If approved, we will issue you with an exemption certificate, and an Exemption Notice to display prominently in the vehicle.

You can appeal to the magistrates' court within 28 days if we decide not to issue you with an exemption certificate
